We serve Home Care patients within a 25-mile radius of Northfield. You’ll work closely with others to manage the care of your clients to ensure the needs of your client and their families are being met. This may include collaborating with their Physician, Home Health Aide, Social Worker, Chaplain and others.
As a Home Health RN, you will be responsible for planning, developing and delivering care to patients in their home environment by utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. Effectively interacts with patients, significant others, physicians and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency. The nurse establishes the HHA plan of care as well as indirectly and directly supervising the plan of care per regulations. 

• Current Registered Nurse license with the Minnesota Board of Nursing, in good standing
• BLS certification; or willingness to obtain within 6 months (company paid)
• Initial Crisis Prevention Certification(CPI) or willingness to obtain within 6 months (company paid)
• Valid Driver's License
• Reliable vehicle with proof of insurance
• Preferred 3-5 years of nursing experience in Med/Surg, Home Care, or Community Health Nursing

Schedule:

• 64hrs/2wks; scheduled various days of the week, Monday-Friday, typically between 8:00am-4:30pm
• Rotating call schedule for evening/nights, every 9th weekend, 8:00am-4:00pm
